You are required to read the following information very carefully and make sure that you understand it fully before participating in, or allowing your child to participate in any activity or program at the Weston Art & Innovation Center.

Liability Waiver, Release, Indemnification and Hold Harmless Agreement
You are required to read the following information very carefully and make sure that you understand it fully and sign it before participating in, or allowing your child to participate in, this activity or program.

I agree to abide by the Weston Art & Innovation Center (AIC) Policy. I am fully aware that participation in Weston AIC programs and activities involves use of inherently dangerous equipment and carries with it the risk of bodily injury or harm to myself and/or my child/children. I, on my own behalf and/or on behalf of my minor child, [name of child] hereby agree to forever release and hold harmless the Town of Weston, the Weston Library, Weston MA, and their officers, employees, volunteers, staff, committees and boards (collectively “Releasees”), from and against any and all liability, loss, damages, expenses, claims, or actions (including costs and reasonable attorneys’ fees) that may have arisen in the past or may arise in the future, directly or indirectly, for bodily injury and/or property damage. .

I also promise to indemnify, defend and hold harmless the Releasees on behalf of myself and/or minor participant against any and all legal claims and proceedings of any description that may have been asserted in the past, or may be asserted in the future, directly or indirectly, arising from bodily injuries or damage to any property resulting from my/my child/children’s participation in the Weston AIC Program. I also promise to fully reimburse the Releasees for any loss or damage as a result of the participant’s involvement in said programs.

I have read and understand this release, indemnification and hold harmless form and I voluntarily sign it. I understand that participation in this program is entirely voluntary and that I am/my child/children is/are free to choose not to participate in said program or have a minor participate in said program. By signing this form, I authorize participation in the Weston AIC Program with full knowledge that the Releasees will not be liable for any damage or injuries resulting from said program.

I hereby give permission to the Weston AIC for emergency transportation and/or treatment in the event of illness or injury to myself or my child/children, and this release extends to any liability arising in connection with such transportation and/or treatment. I hereby accept responsibility for the payment of any emergency transportation and/or treatment. I further certify that I am/my child/children is/are in good physical condition, and have no medical or physical conditions that would restrict my participation in this activity or program.

COVID Related Requirements:
We follow state and local guidelines. At this time masking is optional.

I agree that the Weston Art & Innovation Center may use my or my child or ward’s image or video from the above-named event and that the images or video may be broadcast, published, and/or electronically distributed without limitation including rebroadcast to all public access centers, social media platforms, and websites through any means available and I shall not receive any compensation for my or my child’s or ward’s participation. I also agree to the use of my name, likeness, portrait or pictures, voice and biographical material about me or my child/ward without limitation for organizational and/or promotional purposes. I further agree that my or my child’s or children’s or ward’s participation confers upon me no rights to use and no ownership or copyright. I release the Weston Art & Innovation Center and its agents, employees and assigns forever from all liability which may arise from any and/or all claims by me or any third party in connection with my participation. It is understood that the Weston Art & Innovation Center is under no obligation to broadcast or publish in any format the images or video from the above-named event.
